GETTING A BIRTH OR DEATH CERTIFICATE

Birth Certificates 

• The office only issues certificates for births that occurred in Jefferson County Indiana from 1882 – present. 
• For births outside of Jefferson County Indiana, please contact the county in which the event occurred. 
• For births outside of the State of Indiana, please contact the state in which the event occurred.

New Birth Certificates 

• Certificates for new births typically takes up to 2 weeks to become available. While the Jefferson County Health Department strives to provide this service as quickly as possible, the information must first be received from the birthing hospital (or center) and uploaded into State of Indiana records system.
• The Office of Vital Statistics processes orders within 2 business days once the information is available.

Death Certificates 

• This office only issues certificates for deaths that occurred in Jefferson County Indiana from 1882 – present.
• For deaths outside of Jefferson County Indiana, please contact the County in which the event occured.
• For deaths outside of the State of Indiana, please contact the state in which the event occured.

Attention Applicants 

• If you are applying for your own Birth Certificate and you have changed your name since birth, (Marriage, Legal Name Change, Adoption) please enter the reason for your name change in the 'Special Instructions' field on Step 4 of the application.
